When Are Probiotics Worth Taking?
Let's start with an important point: probiotics are not essential for healthy people who already eat a varied diet rich in plants and naturally fermented foods.
They become genuinely valuable when the gut microbiome is under pressure or when a consistent daily habit helps support a weakened digestive ecosystem over time. Understanding the difference between targeted use and daily maintenance helps clarify when probiotics are most relevant.
During and After a Course of Antibiotics
This is by far the best-studied situation, and probably the most important one. While antibiotics eliminate the bacteria responsible for an infection, they also destroy a significant proportion of the beneficial bacteria living in the gut.
The French National Health Insurance (Assurance Maladie) notes that antibiotic treatment can alter the intestinal microbiome for several months after the medication has ended³. One of the most common consequences is antibiotic-associated diarrhoea, which affects around one in five patients.
A Cochrane systematic review, considered one of the highest standards in evidence-based medicine, concluded that taking probiotics alongside antibiotics significantly reduces this risk, particularly in children and adults undergoing prolonged antibiotic treatments⁴.
The practical recommendations are straightforward:
- start probiotics on the first day of antibiotic treatment
- leave at least two hours between the antibiotic and the probiotic
- continue taking probiotics for two to four weeks after finishing the antibiotics to support microbiome recovery.
After Gastroenteritis or a Digestive Infection
A viral, bacterial or parasitic gastrointestinal infection can severely disrupt the balance of the gut microbiome and leave behind a persistent imbalance known as post-infectious dysbiosis. This dysbiosis is recognised as one of the mechanisms involved in the development or worsening of irritable bowel syndrome (IBS) after gastroenteritis.
Taking probiotics during the days and weeks following the acute infection may help restore a healthier microbiome and reduce long-term digestive symptoms. The strains that have been studied most extensively in this context are Lactobacillus rhamnosus GG and Saccharomyces boulardii.
When Travelling: Preventing Traveller's Diarrhoea
Travelling often means sudden changes in diet, water, daily routine and exposure to unfamiliar microorganisms. Traveller's diarrhoea remains one of the most common digestive issues experienced abroad.
A review published in the international scientific literature found that taking targeted probiotics a few days before departure and continuing throughout the trip significantly reduces the risk of developing traveller's diarrhoea⁶.
The strains with the strongest evidence in this setting are L. rhamnosus GG and S. boulardii. This preventive strategy is especially relevant for high-risk destinations, longer trips and people with sensitive digestive systems.
During Periods of Prolonged Stress
Chronic stress is not just a psychological issue—it has measurable effects on the gut microbiome. A review published in PMC summarised how prolonged stress alters the composition of the gut microbiota, increases intestinal permeability and amplifies digestive discomfort through the gut-brain axis⁷. During demanding periods—heavy workloads, bereavement, major life changes or exams—daily probiotics cannot replace stress management, but they may help reduce some of its digestive consequences while supporting a microbiome under pressure.
In these situations, probiotics should be viewed as supportive nutrition, to be continued for as long as the stressful period lasts.
During Seasonal Changes
The transition from summer to autumn and from winter to spring brings changes in diet, climate and immune function. The gut microbiome also changes during these periods. Dietary diversity often decreases in winter, respiratory infections become more common, daylight exposure drops and physical activity tends to decline.
These seasonal transitions are therefore an appropriate time to support the microbiome with probiotics, particularly for people who experience frequent winter infections or seasonal allergies.
During Pregnancy, Breastfeeding and Perimenopause
Major hormonal transitions are accompanied by well-documented changes in the intestinal, vaginal and skin microbiomes. During pregnancy, certain probiotic strains have been investigated for their potential role in supporting vaginal health, reducing the risk of gestational diabetes and helping shape the newborn's microbiome.
During perimenopause, declining oestrogen levels can alter the gut microbiome and contribute to bloating, abdominal weight gain and changes in bowel habits. Combining probiotics with a diet rich in fibre and polyphenols may provide valuable support. As always during these life stages, medical advice is recommended to choose the most appropriate strains.
For Endurance Athletes
Long-duration endurance exercise—running, cycling or trail running—places considerable stress on the intestinal barrier. During prolonged exercise, blood flow is redirected towards the muscles, temporarily reducing oxygen supply to the digestive tract.
The result can include bloating, intestinal cramps and digestive discomfort during or after exercise, a phenomenon commonly referred to as runner's leaky gut. Daily probiotic supplementation, particularly with strains that support the intestinal barrier such as Lactobacillus plantarum, is now included in the nutrition strategies of many endurance athletes.
For Chronic Digestive Disorders
Irritable bowel syndrome, chronic bloating, functional constipation and functional dyspepsia are all conditions in which specific probiotic strains have demonstrated clinical benefits. The World Gastroenterology Organisation (WGO) emphasises that probiotic strains should always be selected according to the condition being treated. A probiotic that works for traveller's diarrhoea will not necessarily be effective for IBS, and vice versa⁸. In these situations, probiotics should generally be taken for at least four to twelve weeks before assessing their effectiveness.
As Part of Everyday Gut Health
For healthy people who simply want to support their microbiome on a daily basis, the strongest evidence still points towards a diet rich in fermented foods, prebiotic fibre and polyphenol-rich plants. The French National Institute of Health and Medical Research (Inserm) reminds us that consistency matters far more than intensity in maintaining a healthy microbiome².
A probiotic shot every morning—or a daily serving of fermented yoghurt—will have a greater long-term impact than completing a one-month course followed by several months without probiotics.
